What You Need to Know About Robotic Prostate Cancer Surgery

Robotic Prostate Surgery

Robotic Surgery for Prostate Cancer

One of the minimally invasive treatments for Localized Prostate Cancer is the Robotic Surgery for Prostate Cancer. Also known as da Vinci prostatectomy, this involves highly advanced robotics along with state-of-the-art computer technology. It involves less bleeding, less pain, minimal stay at the hospital and faster recovery. It works very well for people who are in the early stages of prostate cancer.

How does Robotic Prostate Surgery Work?

It requires a special kind of training for doctors. There are two machines here:

  • The control unit
  • The patient unit

The control unit is handled by the surgeon. It controls the patient unit and offers the surgeon a 3D view of the operating area. The surgeon can magnify this about 10 to 12 times.

The patient unit will be beside the operating table. It has four arms. The camera is held by one arm and the other arms are for holding the surgical instruments.

The surgeon creates five incisions in the abdomen, into which the camera and other instruments are inserted for the prostate cancer surgery. With the help of the master controls in the control unit, the surgeon moves the instruments on the patient unit.

Any movements that a surgeon makes get turned into smaller movements by the machine. They will be tiny and accurate, without any shaking. With robotic assistance, it becomes easier to make cuts and stitch them up.

Eligibility for Robotic Surgery for Prostate Cancer

Robotic surgery is one of the top prostate cancer treatment options. It can be very effective for localized prostate cancer where cancer has not spread outside the gland and is at an early stage. It removes the cancerous tissue/tumor with a layer of healthy cells (called a clear margin) to make sure there are no remnants that can cause a relapse or recurrence.

Robotic Surgery Side Effects & Risks

Erectile Dysfunction and Urinary Incontinence are the two main side effects of Robotic surgery for prostate cancer. However, these are temporary by nature. The other side effects such as bleeding and pain are a lot lesser when compared to the other types of prostate surgeries.

However, these risk & side effects vary from person to person depending upon the patient’s age, overall health, the stage of cancer and the expertise of the surgeon in performing robotic prostatectomy.

Apart from these, you can come across side effects such as:

  • Abdominal Distention, bloating or constipation
  • Bladder Spasms
  • A little bit of blood in your urine
  • Bruising Around your wound sites
  • Swelling in your Ankle or Lower Leg
  • Perineal Discomfort (The area between scrotum and rectum)
  • Penile or scrotal swelling and bruising

While the above are normal side effects of the surgery, there are certain signs that might require immediate medical attention. These include:

  • Fever of 101°F
  • Redness, swelling or too much of smelly discharge from your abdominal surgical incisions
  • Too much of abdominal distention, pain, nausea or vomiting
  • Swelling or pain in one of your legs or calf muscles
  • Shortness of breath or chest pain
  • The decrease in output of your urine
  • Inability to urinate

Preparing for Robotic Prostate Cancer Surgery

There are some pre-testing procedures you may have to go through before undergoing robotic prostatectomy as treatment for prostate cancer. These may include urine analysis, urine culture, EKG, Chest x-ray and various other diagnostic tests based on your requirements.

You will be asked to stop taking aspirin and other non-steroidal anti-inflammatory drugs like Advil, Ibuprofen, Naproxen Aleve and even vitamin supplements about 10 days before the day of surgery.

Plavix & Coumadin are also not to be taken during this period. In case you are on any other medications for problems like asthma, thyroid and high blood pressure, you may take them with a sip of water before you go for the surgery.

It is better not to take your diabetic medication on the day of surgery. It is better to stick to a clear liquid diet the day before your surgery. After midnight you cannot have anything until your surgery is over.

What to Expect after HIFU?

You will probably be discharged from the hospital 24 hours after the surgery if everything is fine. You may, however, need to ask someone to pick you up from the hospital as it is not advisable for you to drive with the catheter in place. This is going to be there for a week after which the doctor will remove it.

You cannot go back to your regular diet until you have a proper bowel movement after the prostate surgery.  You should, however, avoid gassy food items such as cabbage, beans, broccoli, carbonated drinks such as seltzer, soda and ginger ale and anything that is spicy. Apart from these, you will have to keep in mind the following things for a period of six weeks from the day of your prostate cancer treatment surgery:

  • You cannot push or strain during bowel movements
  • Do not go swimming or sit in a bathtub
  • Do not lift heavy weights
  • Do not take part in sports activities
  • Do not exercise
  • Keep changing your sitting position every 45 minutes or so
